What Causes Brain Fog? 4 Root Causes Worth Exploring

1. Hormones and Brain Fog
Hormones carry messages between your brain and body. When those signals change, your focus, memory, mood, and energy can change too.
Thyroid Function
Thyroid hormones help regulate how your brain uses energy. When thyroid function is low, you may feel physically tired and mentally slower.
People experiencing brain fog with hypothyroidism commonly report fatigue, low mood, forgetfulness, and difficulty concentrating.
Perimenopause and Menopause
Estrogen interacts with areas of the brain involved in memory, attention, and mood. As estrogen fluctuates during perimenopause, some women notice trouble finding words, staying focused, or remembering details.
Sleep disruption, hot flashes, stress, and depression may also contribute to cognitive changes during perimenopause.
If your brain fog appeared alongside menstrual changes, hot flashes, or disrupted sleep, hormones may be one piece of the picture.
2. Nutrition, Gut Health, and Brain Fog
Your brain needs nutrients and steady energy to think clearly. If either is missing, keeping up with your day can feel much harder.
Nutrient Gaps
Low levels of vitamin B12, iron, folate, or vitamin D may contribute to fatigue and cognitive symptoms. Not eating enough protein or overall calories may also affect mental energy.
Vitamin B12 deficiency, for example, can cause fatigue and neurological symptoms.
Blood-Sugar Regulation
Your brain needs a steady supply of energy. Long gaps between meals or meals low in protein and fiber may leave you feeling shaky, irritable, or unfocused.
If your brain seems to “clock out” after lunch or you often experience energy crashes, your eating patterns may offer a useful clue.
Gut Health and Absorption
Eating nutritious food is important, but your body also has to digest and absorb it.
Digestive conditions, ongoing inflammation, restricted diets, or gastrointestinal surgery may increase the risk of nutrient deficiencies. Brain fog may appear alongside digestive symptoms, low energy, or difficulty eating regular meals.
3. Stress, Sleep, and Mental Health
Your brain cannot perform at its best when it is constantly managing stress, running on poor sleep, or struggling to regulate attention and mood.
Chronic Stress
Chronic stress can feel like having too many tabs open in your brain. You’re trying to focus, but deadlines, worries, and other demands keep running in the background.
Over time, that mental load can affect your concentration, decision-making, sleep, and energy—all effects of chronic stress noted by the CDC.
Sleep
Sleep gives your brain time to recover and organize information. The CDC notes that adequate sleep supports attention and memory. If your sleep is short, disrupted, or unrefreshing, thinking clearly may feel harder.
If you wake up feeling as though your brain never fully powered down, sleep deserves a closer look.
Mental Health and Attention
Anxiety can make it harder to filter distractions, depression may slow thinking and reduce motivation, and ADHD can affect attention regulation, working memory, and task completion.
These experiences can overlap across conditions, so difficulty focusing alone does not tell us why it’s happening. Understanding which factors are contributing helps guide your care.
4. Environment and Daily Exposures
Sometimes the most useful clue is not just how you feel, but what happened before your symptoms appeared.
Environmental Toxins
Indoor air quality, smoke, mold, poor ventilation, and chemical exposures may affect health.
The National Institute of Environmental Health Sciences identifies indoor and outdoor air pollution as concerns for neurological and other body systems.
This does not mean every case of brain fog comes from an environmental exposure. It simply means your surroundings deserve attention, especially if you consistently feel better away from a particular environment.
Medications and Substances
Some medications can affect alertness, memory, or concentration. Alcohol, cannabis, and other substances may also influence sleep and cognitive function.
A complete review should include prescriptions, over-the-counter medications, supplements, alcohol, cannabis, and other substances. Even something you have taken for a long time may be worth reviewing with your clinician if the dose or combination has changed.
